[MinnowBoard] MinnowBoard Max - no Input signal on screen

Sounds like the first one I bought (still dead). I had to send it back, they re-flashed it and sent it back. It worked for a while then went dead again. If you have a SPI flasher, you can try to re-flash the board (SPI Hook TinCanTools.com or DediProg SF 100, the first one is much cheaper). However, this did not work for mine.

I only pressed the power switch after I tried different monitors and power supplies (I misstated the rated amperage of the power supply I am currently using - it is 3A instead of 2.5A).
I just turned it on - left it on for a minute then pressed the power switch - nothing appeared to happen the one LED that was on (D1 - next to the button) did not flicker - I have never seen D2 go on.

Did you press the power button near the blue LEDs?
Sent from my iPhone. Typos are Apple's fault.

I have been trying to follow the setup procedures for a new MinnowBoard Max as described on:
http://wiki.minnowboard.org/MinnowBoard_MAX_HW_Setup
I have a 2.5Amp AC to 5V DC power supply (positive on the inside of the barrel) that shows a voltage of 5.4V without a load (just using a multi-meter) and have tried several monitors but each of these reports no signal input.
The board shows only has one illuminated led light when power is applied - I believe that there should be two?
Is the board defective or should I be doing something else (it was purchased recently from Mouser).
Thank you in advance for any help.
Rico
