[Elinux-wiki-dev] php error when saving a page for the first time

i tried editing a page, and after hitting save, it took me back to the
edit page and this was at the start of the generated html output
(before the <!DOCTYPE> and <html> tags):

after hitting save a few more times and seeing the same error, i
noticed the simple captcha where i had to enter "What is the name of
the Linux mascot?".  putting this above the wiki text and not next to
the "Save" button is fairly unintuitive.  i'd suggest it get moved
down next to the save button and/or summary text box.
